Leave of Absence Request Form

 

Parents are strongly advised to take holidays during the school holidays. If you wish to request a period of absence for any reason please complete the attached Leave of Absence Request Form at least four weeks before the start of the proposed leave (if possible).

The completed form should be returned to the school office who will then pass it on to the Headteacher for consideration.
